A Step-by-Step Guide to BUA Amendments

Location Change Form

  1. List all the spaces that you have added as your workspace (check Add), modified the use of (check Modify), or will not be using anymore (check Delete). Include all the lab spaces you will continue to use for the same purpose without checking the Add, Delete, or Modify box.

    1. If you will only be updating your PI, just change that information at the top of the form and list all of your current spaces without indicating any actions.


2. If moving to a new Biosafety Cabinet for Tissue Culture work, include that information here. If you will be using the shared Tissue Culture facilities at Bakar Labs, you can copy the information below into the form.

Experimental Change Form

In this form, you should describe the risks and mitigations of the proposed work, including how waste, spills, exposures, transport, and post-work clean-up are addressed. The descriptions should be complete enough to be understood on its own, and if previously approved work is needed for context, the application should clearly delineate between what has been previously approved and what is being added.


Section 1 should include relevant information, including name of PI, Title, etc., and information about your original BUA form. In Section 2 you should cover the personnel doing this new work, and give the dates of any new trainings required by the new work.


Sections 3 & 4 deal with the actual materials being added to your experimental procedure. For example, if you were adding a new cell line, it might look like this:


For Section 4 specifically, you will need to include the following information about the recombinant DNA you will be using:

Host: add any new organisms - virus, bacteria, fungus, parasite, human cell lines, other cell lines, human primary cells, other animals’ primary cells, etc.

Vector: vectors, including plasmids and lentivirus

Nature of inserted sequences: for example, marker, gfp cDNA, antibiotic resistance, ampicillin and kanamycin

Source of inserted sequences: depending on each inserted sequence, for example GFP comes from some Aequorea victoria

Types of manipulation: for example, standard tissue culture, growth of worms occur using E-coli agar gel plates, transformation, electroporation, etc.

Attempt to express foreign gene: yes, AmpR, KanR, bacterial resistance, gfp

Protein produced: for example, Green Fluorescent Protein, list all or send a list as attachment if too many

Containment: BSL1 or BSL2

Section 5 only applies to experiments using animal subjects, which are not permitted in Bakar Bio Labs. In Section 6you will need to write a paragraph summarizing the changes you indicated in the previous sections. Make sure to include how waste, spills, exposures, transport, and post-work clean-up are addressed, and make the differences between your existing work and the new work are different.




Once either form is completed, make sure to follow the instructions on the main BUA Amendment article about how to submit the form and go through the approval process.
